Job Description

Doral Health & Wellness is seeking a  Full-Time Field Emergency Medical Technician (EMT)  to join our interdisciplinary care management team. This mobile role involves providing in-home support to patients across all five boroughs of New York City, as well as Westchester and Nassau counties. You will assist with basic clinical assessments, collect vital health data, and support the coordination of care services for individuals with chronic conditions.

Key Responsibilities:

• Conduct routine in-home visits to support patient care management plans
• Perform basic clinical tasks such as monitoring vital signs, glucose levels, and medication compliance
• Document and report patient conditions to the supervising nurse or care manager
• Assist patients in navigating healthcare services and reinforcing care instructions
• Maintain accurate records and uphold patient confidentiality (HIPAA compliant)
• Travel daily across assigned territories in NYC and surrounding counties

Qualifications:

• Current  New York State EMT certification (Required)
• Valid driver's license and ability to travel throughout NYC, Westchester, and Nassau
• Previous experience in home care, field work, or care coordination preferred
• Strong interpersonal, communication, and organizational skills
• Able to work independently while maintaining close communication with the care team

Work Setting:

• Field-based (home visits)
• Day shifts only: Monday to Friday, 9 AM – 5 PM
• No weekends or overnight shifts
